General Summary:


The Director of Nursing will report to the Administrator of Health Services and is responsible for the overall supervision, provision, and quality of nursing care in the Health Center.  The DON is responsible for the selection, training, discipline and supervision for all nursing related Health Center personnel and is responsible for the procurement of appropriate supplies and equipment and operating within the departmental budget.


 


Principle Duties


 


Essential Job Duties:


·        Responsible for the development, organization and operation of nursing services for the Health Center.


·        Responsible for the coordination of nursing services of Health Center residents including the procurement of outside sources such as: Home Health Care Services, private duty, hospice, rehabilitation services, psychological, etc. necessary to ensure that resident’ needs are met.  Communicates and meets with outside consultants as necessary to assess and make recommendations for resident care needs.


·        Responsible for the coordination and direction of the total planning for nursing services including recommendation through a staffing plan of the number and levels of licensed and certified nurses’ aides to be employed (i.e.  Approving staff Request for time off and hours worked for Payroll processing).


·        Maintains an effective restorative care program in collaboration with therapy provider.


·        Ensures completion of department checklist for all nursing staff upon hire.



  • Selects, trains, evaluates, disciplines all nursing services personnel. Ensures completion of written orientation evaluations and annual performance evaluations of all nursing staff.


·        Responsible for developing and maintaining a schedule of daily rounds and assignments of duties for all nursing staff to assure 24-hour coverage sufficient to meet federal and state regulatory requirements and Facility standards of care.


·        Develops appropriate action plans for quality improvement (i.e. staff and department processes) and is responsible for follow through.


·        Implements LCS Standards as demonstrated through the use of resources and programs.



  • Maintains clinical skills and stays knowledgeable of current standards of nursing practice through nursing education related to geriatrics.


·        Ensures that significant clinical developments of residents are reported to their families and/or responsible party, physicians, the Medical Director, the Administrator of Health Services and State, as necessary.


·        Review nursing requirements of each resident admitted to the Health center and assist the attending physician in planning care. Oversees the care plan development for residents.


·        Effectively manages nursing staff within the area of responsibility, providing direction and setting expectations consistent with the goals of ECP.


·        Maintains close coordination with all departments to ensure continuity and collaboration of services.


·        Regularly audits clinical systems and processes to promote and ensure compliance.


 


Other Duties:


·        Appoints, with the Administrator of Health Services approval, a nurse to act in his/her behalf during temporary absences.  Designates a nurse in charge each shift.


·        Participates in the coordination of resident care services through departmental staff meetings.  Attends, prepares and analyzes necessary information for the Quality Assurance Process Improvement Meetings and annual policy review.


·        Recommends the modification, addition, or deletion of nursing policies.


·        Develops and maintains a statement of philosophy and standards of care that outlines the quality of nursing and medical care that the Facility provides.


·        Develops and maintains written nursing standards of practice.  Such standards shall include, but not be limited to, defining routine care to be provided by licensed and certified nurses’ aides and determination of when more care is needed.


 


·        Develops and maintains nursing service policies and procedures to implement the standards of care.  This shall include the coordination and integration of nursing services with other resident care services.


·        Participates in planning an annual budget for department.  Ensures monthly expenditures for department remain within budgeted allowances.  Reviews monthly operating statement for accuracy and explains variances to budget.


·        On-call rotation is required.


·        All employees are expected to respond to emergency situations involving the safety of residents, other employees and our physical plant.  This includes the ability to assist with a possible evacuation of residents.


·        All employees are responsible to adhere to all facility policy and procedures as they relate to compliance with the Health Insurance Portability and Accountability Act (HIPAA) and Medicare Compliance.  All employees are responsible to ensure that any Protected Health Information and Medicare Compliance Information that he/she comes in contact with is maintained in a confidential manner.


·        Abide by emergency, safety and health regulations. Eliminate or report all hazardous conditions as they occur.


·        Abide by all rules and policies set forth by the company and department.


·        Attends in-service training and education session, as required.


·        Employees are responsible to perform any other duties as assigned.

Qualifications:



  • Associates or Bachelor’s Degree in Nursing.

  • Prior experience and experience in long-term care facility, preferred.


·        Must have Registered Nurse license in the State of WI.


·        Ability to understand and follow instructions in English, communicate effectively and perform simple arithmetic.  Ability to document care given.


·        Possesses ability to communicate effectively with residents, families and staff.  Must use sound judgment, works well under pressure and possesses strong organizational skills.


 


Physical Requirements/Working Conditions:


Frequent standing, stooping, bending, stretching, squatting; must be able to stand/walk for up to 70% of the work day; must be able to lift , transfer, turn and position residents with assistance. May be subjected to offensive odors and combative behavior.  Must be able to lift and carry up to 20 lbs. frequently and 50 lbs. occasionally.  May be exposed to communicable diseases including the HIV and HBV viruses.  Must be able to work paying close attention to detail with frequent interruptions.


Equipment Utilized:


Medication cart, call and alarm systems, sphygmomanometer, glucometer, computer, blood pressure cuff, suction machine, tub lifts, residents beds, transfer/repositioning devices, feeding pump, IV pump, scale (and anything else related to resident care).



Qualifications:



  • Associates or Bachelor’s Degree in Nursing.

  • Prior experience and experience in long-term care facility, preferred.
